a typical plan, the employer agrees to pay a portion of
eligible dental expenses that may look like this:

Patients see the dentist of their choice and receive the
treatment as diagnosed by their doctor. The claim is sent
to Direct Dental for professional review and benefit determination.
The claim check is drafted and sent to the employer for
signature and disbursement to employee or dentist. Benefits
are paid from the employer's account only after treatment
has been completed and only for those employees who receive
dental care. Our clients receive responsive phone support
and monthly reporting of utilization and employee eligibility.
As
you'd expect, the more restrictive the plan is, the lower
the costs will be to the employer.
Each of our clients has the option to add these cost controlling
features to the basic design to decrease plan expenses:
Deductible:
0, $25 or $50 annually
Annual Maximums: $500 to $2000 per person or family
Waiting Periods: none to 12 months for specific services
Orthodontic coverage available, even for adults
Prepayment by patients or assignment to providers
